The School Code of Illinois requires that each student entering 9th grade have a physical examination completed by a licensed physician, advanced practice nurse, or physicians assistant. Class schedules will not be issued without a completed physical and an up to date immunization record on file in the nurses office. Please be sure ALL sections are completed and signed. Return completed forms to the nurses office by July 1st. For your convenience, see below for the Certificate of Health Exam as well as the Medication Authorization/Permission form which you may print to bring the doctor's office. Thank You.
